HIV, STI, and viral hepatitis outbreaks often require local health departments (LHDs) and their partners to rapidly expand and coordinate activities across surveillance, case investigation, outreach, prevention, clinical care, and supportive services. Without clear roles, communication structures, and referral pathways, people may experience delays or become lost to follow-up as they move between organizations and services.
This new NACCHO brief outlines practical strategies for strengthening coordination at both the systems and patient levels. It explores how LHDs and their partners can establish effective coordination mechanisms, facilitate collaborative problem-solving, map services and partner roles, and strengthen referral pathways and case coordination.
